Please apply. Once you have been approved, you'll be prompted to fill out the Registration Form. Please fill out one per family. 

REGISTRATION FORM

Please fill out one form per child. Turn into the office or email them in.

IMMUNIZATION RELIGIOUS EXEMPTION FORM

Fill out and turn into the office with the children registration form or turn into Mat-Su Central if your child is enrolled with them (they have notary in the building) 
One form per child.
